Abstract
Background: A Medication administration error [MAE] is any preventable act that contributes
to the failure of proper medication use in the treatment process resulting in harm for the patient
to the extent of disability and death. A Medication error listed in the top ten causes of mortality
worldwide. Globally, Medication errors [MEs] present a substantial contribution to ill health, and
even death and are listed as one of the five medical error categories classified by the American
Institute of Medicine in Ethiopia. An institution-based, cross-sectional study conducted in two
public hospitals in Southern Ethiopia indicated that 71% of nurses made MAE. However,
however, only a few relevant studies were conducted in Ethiopia to assess this problem.
Objective: To assess the magnitude and contributing factors of medication administration errors
among nurse on, Gurage zone, Ethiopia, 2023.
Methods; Crosse sectional study design was conducted. The study includes 348 nurses from four
hospitals nurses were selected by systemic random sampling technique. Data was cleaned,
checked for inconsistencies and missed values, coded and entered for analysis to SPSS (SPSS
Inc. version 25).multi variable logistic regression was done to identify factors associated with
medication administration errors.
Result; A total of 320 nurses participated in this study, with 91.9% response rate. Of which
219(68.4%) nurses made Medication administration error in the last 12 months. In this study
medication administration error was significantly associated with unavailability of guideline for
MA [AOR=1.64; 95% CI (1.01, 2.64)], interruption during medication administration [AOR =
1.94, 95 % CI (1.12, 3.36)], low nurses to patient ratio [AOR = 5.19 % CI (1.74, 15.41)], and
administering medication during night time [AOR = 2.75% CI (1.11, 6.84)].
Conclusion and recommendation: Medication administration error was highly prevalent in four
selective hospitals in Gurage zone. This study showed that there is a significant association
between medication administration error and factors such as Availability of guideline for MA,
work load, interruption during MA, nurse to patient ratio, and Night shift. Develop appropriate
guidelines for safe medication administration, minimizing distracters during medication
administration, increase nurse to patent ratio and work load would be helpful for minimizing
medication administration errors
